Living Programs
Feels Like Home
Lambs Farm’s residential program is designed specifically to provide a nurturing environment for our participants while promoting self-reliance. We offer a variety of housing options, both on the farm and in the community, to better meet each participant’s needs.
Services for Residents
Campus Group Homes
Residents of our nine campus group homes learn living skills in a supportive environment. With staff support, housemates share meal preparation and housekeeping responsibilities while enjoying the company of other residents. Residents at Lambs Farm have several options for housing based on their specific needs and levels of independence. Housing includes our Group Homes for multiple participants, individual apartments, and assisted care residence.
Community Residences
Located in surrounding towns, Lambs Farm’s community residences are homes for those looking for a more independent lifestyle off campus. Residents in these homes receive regular staff support and have access to all Lambs Farm services.
Transportation Services
We provide extensive transportation services to help participants go to work and attend appointments off-campus. On-campus transportation between buildings is available for participants during inclement weather and as needed for those with limited mobility. In the absence of public transportation, we offer the Rainbow Run, a van that takes participants to community locations like stores, banks, and the library. Participants who are unable to run errands independently have opportunities to do so with staff support. We also provide transportation to help participants attend events in the community.
Case Management
Each participant who lives at Lambs Farm has a case manager. Each case manager meets the state and federal requirements for a Qualified Intellectual Disabilities Professional (QIDP) as well as Direct Support Professional (DSP) and medication certification. The case manager assists the participant to identify, plan, and achieve their chosen outcomes. The case manager oversees implementation, monitoring, and revision of outcomes-based services in accordance with the state and federal guidelines. The case manager also acts as the liaison with guardians, families, Independent Service Coordination (ISCs), and other outside resources.
Nutrition Services
Nutrition services are available to all participants. Lambs Farm places an emphasis on better-for-you foods, beverages, and snack choices. Seasonal menus are provided by our own in-house dietitian in all the residential areas to ensure a variety of wholesome foods to support good health.
Independence with
Home-Based Support
Lambs Farm’s Home-Based Support & Self-Direction Assistance program empowers individuals with intellectual disabilities to live with their families or semi-independently while receiving the essential supports they need. Our goal is to help each participant and their family navigate resources, manage services, and maximize their independence.
Budget Planning & Management
Helping families create, maintain, and monitor each client’s annual budget.
Coordinating Services
Facilitating day programs, behavior services, and Individual Service Coordination (ISC) to ensure an effective and seamless experience.
Navigating Referrals
Assisting with Department of Rehabilitation Services (DRS) referrals to access additional resources.
Meeting Requirements
Providing support in completing annual documents and fulfilling state-mandated requirements.
Accessing Additional Support
Helping families apply for emergency funding and one-time funding for essential items like adaptive equipment, home and vehicle modifications, and assistive technology.
