HBS Case Manager
The HBS Case Manager is responsible to help families and Participants of the Home Based Support Services program to direct and manage services. They also provide case management services for a group of Lambs Farm Participants who work in community employment settings.
A Bachelor’s degree in Education or Human Services plus one year experience working directly with people with intellectual/developmental disabilities is required. Case Management experience preferred. The person selected must be QIDP approved or able to complete the QIDP training within 6 months of employment. Lambs Farm will provide training as needed. A valid driver’s license and a good driving record are required.
This position can be hybrid, with part of the work being done in the office and part remote, after an initial training period.
